Age- and sex-specific patterns in the prevalence of metabolic syndrome components in the Republic of Korea, 2014–2023: a repeated cross-sectional study of national health screening statistics
Yu-Mi Jang, Woo-Hyeon Son
Received February 22, 2026  Accepted August 3, 2026  Published online September 23, 2026
DOI: https://doi.org/10.24171/j.phrp.2026.0195    [Epub ahead of print]
Objectives
This study described 10-year trends in the prevalence of 5 metabolic syndrome (MetS) components and examined age- and sex-specific patterns in the Republic of Korea using aggregated national health screening statistics from 2014 to 2023.
Methods
This repeated cross-sectional study used publicly available aggregated data from the National Health Screening Statistical Yearbook of the National Health Insurance Service. Annual prevalence estimates were summarized overall and by age group and sex. Linear regression descriptively summarized temporal trends. Because of a marked reduction in examinees in 2018 and a discontinuity around the 2018–2019 transition, sensitivity analyses excluding 2018 were performed.
Results
From 2014 to 2023, the prevalence of abdominal obesity increased from 21.0% to 25.5%, hyperglycemia from 34.7% to 40.1%, and hypertension from 42.3% to 45.1%. Hypertriglyceridemia and low high-density lipoprotein (HDL) cholesterol appeared to decrease over the same period but showed a marked discontinuity between 2018 and 2019. Prevalence generally increased across older age groups, particularly for hypertension and hyperglycemia. Men had a higher prevalence of 4 MetS components, whereas women had a higher prevalence of low HDL cholesterol.
Conclusion
MetS components showed heterogeneous temporal, age-specific, and sex-specific patterns in Korean national health screening statistics. Increases in abdominal obesity and hyperglycemia support the need for earlier age- and sex-tailored cardiometabolic prevention, whereas apparent declines in lipid-related indicators should be interpreted cautiously due to the 2018–2019 discontinuity. Because this study used aggregated repeated cross-sectional data, individual-level longitudinal studies are needed to clarify cardiometabolic risk trends in the Republic of Korea.

Association between park accessibility and multimorbidity in Korean older adults: a cross-sectional study
Min-Ju Seo, Jin Young Nam
Received March 27, 2026  Accepted July 5, 2026  Published online August 26, 2026
DOI: https://doi.org/10.24171/j.phrp.2026.0296    [Epub ahead of print]
Objectives
Adequate physical activity is essential for promoting healthy life expectancy among older adults. Park accessibility is an important environmental determinant of physical activity and may support multimorbidity management in this population. However, evidence on the association between park accessibility and multimorbidity remains limited, particularly in nationally representative Korean samples. This study examined the association between park accessibility and multimorbidity among older Korean adults.
Methods
This cross-sectional study analyzed data from 10,078 individuals aged ≥65 years who participated in the 2023 National Survey of Older Koreans. Multiple logistic regression was used to examine the association between park accessibility and multimorbidity after adjustment for covariates. Stratified analyses assessed this association by physical activity level, and additional logistic regression analyses evaluated associations between park accessibility and common chronic diseases.
Results
The prevalence of multimorbidity among older adults was 64.8%. After adjustment for all covariates, older adults living 30–59 minutes’ walking distance from a park had higher odds of multimorbidity than those living <15 minutes from a park (adjusted odds ratio, 1.33; 95% confidence interval, 1.12–1.59). Among the common chronic diseases examined, hypertension was significantly associated with park accessibility.
Conclusion
Greater park accessibility may support physical activity and green-space exposure and may thereby contribute to multimorbidity management and healthy life expectancy among older adults.

Personalized medicine as a novel therapeutic approach for autoimmune diseases: new insights and future prospects
Autoimmune diseases are caused by dysfunction of the immune system, leading to inappropriate attacks on healthy tissues. Because patients have diverse genetic predispositions and heterogeneous responses to therapy, personalized medicine (PM) offers an opportunity to improve treatment effectiveness. PM uses diagnostic assessments to tailor treatment through individualized medical interventions. PM may improve therapeutic precision beyond traditional trial-and-error approaches, reduce adverse consequences, and improve outcomes by integrating genomic and transcriptomic data. PM considers genetic and molecular landscapes, immunologic factors, epigenetic influences, and environmental exposures to assess treatment response. However, challenges remain related to diagnostic access, the slow pace of biomarker identification, technological limitations, sustained patient engagement, data management, and computational requirements. Nevertheless, continued efforts to improve understanding of disease pathophysiology, gene expression, and immune regulation—together with the application of novel technologies and machine learning—may advance PM-based therapies. Additional opportunities include drug–target modeling and exploratory singlecell–based approaches to clarify patient-specific therapeutic mechanisms. This review briefly introduces the potential of PM for type 1 diabetes, rheumatoid arthritis, and multiple sclerosis.

Current status of hepatitis C treatment and its barriers in Jeonbuk, Republic of Korea
Objectives
In alignment with the World Health Organization’s goal of eliminating hepatitis C, this study assessed the current treatment status and reasons for non-treatment among patients with hepatitis C in Jeonbuk State, Republic of Korea, to inform strategies for improving care engagement.
Methods
Among 311 individuals diagnosed with hepatitis C and reported through the National Notifiable Infectious Disease Surveillance system between January 2023 and June 2024, 208 patients were surveyed after excluding those who had died or could not be contacted. Statistical analyses included the chi-square test, the Cochran-Armitage test for trend, and logistic regression.
Results
Overall, 116 participants (55.8%) reported having received antiviral therapy. Among the 92 untreated individuals, the most common reason for non-treatment was the absence of symptoms (n=23; 25.0%), followed by the burden of drug costs (n=21; 22.8%).
Conclusion
These findings highlight suboptimal treatment uptake and key barriers that may hinder progress toward hepatitis C elimination. Expanding screening and strengthening linkage-to-care strategies, while addressing financial barriers, will be essential to achieving national elimination targets.

Secondary transmission of severe fever with thrombocytopenia syndrome in a hospital setting in Republic of Korea: a retrospective observational study of personal protective equipment use and infection risk factors
Objectives
This study investigated a cluster of secondary human-to-human transmission of severe fever with thrombocytopenia syndrome (SFTS) in a hospital setting, focusing on infection risk factors and the role of personal protective equipment (PPE).
Methods
A descriptive epidemiological investigation was conducted following the death of an index patient with laboratory-confirmed SFTS. A total of 27 close contacts, including healthcare workers and a funeral director, were monitored for symptoms. Suspected cases underwent real-time reverse transcription polymerase chain reaction testing. Clinical features, PPE use, and exposure histories were analyzed. The Fisher exact test was used to assess associations between PPE use and infection. Viral genotyping and sequence analyses were performed to evaluate transmission routes.
Results
The index patient deteriorated rapidly and died after repeated cardiopulmonary resuscitation (CPR), during which 8 secondary cases occurred. Most infections were identified among individuals involved in CPR or postmortem care without adequate PPE. Although not statistically significant, infection rates were higher among those who did not wear masks or who used low-filtration masks. Proper use of gloves, gowns, and goggles was associated with lower infection rates. Cycle threshold values in secondary cases (range, 34–39) were higher than in the index case (14.07), suggesting lower viral loads. Sequence analysis demonstrated 99.6%–100% homology between the index and secondary cases; all isolates were genotype B, indicating direct transmission.
Conclusion
This study provides molecular and epidemiological evidence of nosocomial SFTS transmission. Inadequate PPE use during aerosol-generating procedures likely facilitated infection, underscoring the importance of strict adherence to PPE protocols and reinforced infection control practices.

Association of quantity and quality of fat intake with sleep quality: a cross-sectional study in Iran
Objectives
The association between dietary fat intake and sleep quality remains unclear. This study investigated the impact of both dietary fat quality and quantity on sleep quality.
Methods
In this cross-sectional study, participants completed a food frequency questionnaire and Pittsburgh Sleep Quality Index to assess quantity and quality of fat intake and sleep quality.
Results
A total of 1,904 participants (55% female) were included in the study. Participants in the highest tertile of polyunsaturated fatty acids (PUFA) intake, compared with those in the lowest tertile, had significantly higher total sleep quality scores (odds ratio [OR], 1.27; 95% confidence interval [CI], 1.01–1.62) and longer sleep onset latency (OR, 1.40; 95% CI, 1.08–1.83). Participants with higher animal fat intake exhibited higher total sleep quality scores (OR, 1.44; 95% CI, 1.09–1.89) and increased sleep disturbances (OR, 1.37; 95% CI, 1.02–1.83). Among women, those in the highest tertile compared with the lowest tertile showed increased odds of higher total sleep quality scores with total fat (OR, 1.37; 95% CI, 1.04–1.82), PUFA (OR, 1.41; 95% CI, 1.07–1.87), monounsaturated fatty acids (OR, 1.44; 95% CI, 1.09–1.90), saturated fatty acid (OR, 1.33; 95% CI, 1.01–1.77), and animal fat (OR, 1.58; 95% CI, 1.18–2.11).
Conclusion
These findings suggest that higher intakes of PUFA and animal fat are associated with higher total sleep quality scores. When stratified by sex, higher intake of total fat and unsaturated fats was associated with higher total sleep quality scores in women only.

Subacute sclerosing panencephalitis incidence following measles elimination efforts in the Republic of Korea
Objectives
Subacute sclerosing panencephalitis (SSPE) is a rare but fatal neurodegenerative disease caused by persistent measles virus infection. After a significant measles outbreak in 2000–2001, the Republic of Korea implemented a nationwide measles elimination program, which led to a dramatic reduction in measles incidence. This study aimed to evaluate the impact of these measles elimination efforts on the incidence of SSPE in the Republic of Korea.
Methods
This nationwide, population-based retrospective cohort study identified patients newly diagnosed with measles and SSPE between 2007 and 2022, registered in the Health Insurance Review and Assessment Service (HIRA) and Korea Disease Control and Prevention Agency (KDCA) databases. Population-based incidence rates of measles and SSPE were calculated and compared annually.
Results
A total of 236 measles cases (HIRA data) and 1,168 measles cases (KDCA data), along with 2,736 SSPE cases, were diagnosed during the study period. Measles incidence significantly declined, reaching zero cases in 2021, while SSPE incidence displayed an upward trend, peaking in 2014. The mean age at SSPE onset was 21.2 years, with a marked male-to-female ratio of 13.0:1.
Conclusion
SSPE incidence was remarkably low in the post-outbreak period, likely attributable to successful measles control. This study underscores the critical importance of maintaining low measles incidence through sustained vaccination efforts, preventing SSPE and other measles-related complications.

Periodontitis and non-communicable diseases in a Brazilian population, a cross-sectional study, Vila Velha-ES, Brazil
Objectives
The objective of this study was to examine the hypothesis that periodontal disease is associated with chronic non-communicable diseases. Methods: In this cross-sectional study, we evaluated the periodontal health condition of the population, based on the community periodontal index, as well as the number of missing teeth and the presence of systemic health conditions. We quantified the association between oral health and the presence of chronic diseases using simple logistic regression, adjusting for confounding factors including age, smoking, and overweight. Results: The study population consisted of 334 volunteers, aged between 19 and 81 years. In patients over 45 years old, periodontal disease was found to be significantly associated with hypertension and diabetes. Furthermore, in female patients, periodontal disease was significantly associated with hypertension, diabetes, and cancer. Conclusion: Our findings indicate that periodontal disease is positively and significantly associated with both arterial hypertension and diabetes, independent of potential confounding factors.

Effect of Paxlovid in COVID-19 treatment during the periods of SARS-CoV-2 Omicron BA.5 and BN.1 subvariant dominance in the Republic of Korea: a retrospective cohort study
Objectives
This study was conducted to assess the efficacy of nirmatrelvir/ritonavir treatment in patients with coronavirus disease 2019 (COVID-19), particularly those aged 60 years and older. Using real-world data, the period during which the BN.1 Omicron variant was dominant was compared to the period dominated by the BA.5 variant. Methods: In this retrospective cohort study, data were collected regarding 2,665,281 patients infected with severe acute respiratory syndrome coronavirus 2 between July 24, 2022, and March 31, 2023. Propensity score matching was utilized to match patients who received nirmatrelvir/ ritonavir in a 1:4 ratio between BN.1 and BA.5 variant groups. Multivariable logistic regression analysis was employed to assess the effects of nirmatrelvir/ritonavir within these groups. Results: Compared to the prior period, the efficacy of nirmatrelvir/ritonavir did not significantly differ during the interval of Omicron BN.1 variant dominance in the Republic of Korea. Among patients treated with nirmatrelvir/ritonavir, a significantly lower risk of mortality was observed in the BN.1 group (odds ratio [OR], 0.698; 95% confidence interval [CI], 0.557–0.875) compared to the BA.5 group. However, this treatment did not significantly reduce the risk of severe or critical illness, including death, for those in the BN.1 group (OR, 0.856; 95% CI, 0.728–1.007). Conclusion: Nirmatrelvir/ritonavir has maintained its effectiveness against COVID-19, even with the emergence of the BN.1 Omicron subvariant. Consequently, we strongly recommend the administration of nirmatrelvir/ritonavir to patients exhibiting COVID-19-related symptoms, irrespective of the dominant Omicron variant or their vaccination status, to mitigate disease severity and decrease the risk of mortality.

Risk factors for SARS-CoV-2 transmission during a movie theater outbreak in Incheon in the Republic of Korea, November 2021: a retrospective study
Objectives
We examined factors contributing to the transmission of an acute respiratory virus within multi-use facilities, focusing on an outbreak of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in a movie theater in the Republic of Korea. Methods: This retrospective cohort study involved a descriptive analysis of 48 confirmed cases. Logistic regression was applied to a cohort of 80 theater attendees to identify risk factors for infection. The infection source and transmission route were determined through gene sequencing data analysis. Results: Of the 48 confirmed cases, 35 were theater attendees (72.9%), 10 were family members of attendees (20.8%), 2 were friends (4.2%), and 1 was an employee (2.1%). Among the 80 individuals who attended the 3rd to 5th screenings of the day, 35 became infected, representing a 43.8% attack rate. Specifically, 28 of the 33 third-screening attendees developed confirmed SARSCoV-2, constituting an 84.8% attack rate. Furthermore, 11 of the 12 cases epidemiologically linked to the theater outbreak were clustered monophyletically within the AY.69 lineage. At the time of the screening, 35 individuals (72.9%) had received 2 vaccine doses. However, vaccination status did not significantly influence infection risk. Multivariate analysis revealed that close contacts had a 15.9-fold higher risk of infection (95% confidence interval, 4.37–78.39) than casual contacts. Conclusion: SARS-CoV-2 transmission occurred within the theater, and extended into the community, via a moviegoer who attended the 3rd screening during the viral incubation period after contracting the virus from a family member. This study emphasizes the importance of adequate ventilation in theaters.

Study on Seroprevalence and Leptospiral Antibody Distribution among High-risk Planters in Malaysia
J. Mohd Ridzuan, B.D. Aziah, W.M. Zahiruddin
Osong Public Health Res Perspect 2016;7(3):168-171.
Published online June 30, 2016
DOI: https://doi.org/10.1016/j.phrp.2016.04.006
Objectives
To determine the leptospirosis seroprevalence and to identify the predominant infecting serovars among oil palm plantation workers.
Methods
The cross-sectional study involved 350 asymptomatic oil palm plantation workers in Melaka and Johor. A serological test using the microscopic agglutination test was conducted in the Institute of Medical Research with a cut-off titre for seropositivity of ≥1:100.
Results
The overall seroprevalence of leptospiral antibodies was 28.6%. The job category with the highest seroprevalence was the fruit collector with 59.2%. The predominant serovar identified was serovar Sarawak (Lepto 175) (62%).
Conclusion
A high seroprevalence of leptospiral antibodies was detected among oil palm plantation workers and specifically among fruit collectors. The predominant infecting serovar among the workers was serovar Sarawak (Lepto 175). The findings suggest that more studies are needed to determine the reasons for the high seroprevalence and the transmission and pathogenicity of the local serovar Sarawak (Lepto 175).

Korea Community Health Survey Data Profiles
Yang Wha Kang, Yun Sil Ko, Yoo Jin Kim, Kyoung Mi Sung, Hyo Jin Kim, Hyung Yun Choi, Changhyun Sung, Eunkyeong Jeong
Osong Public Health Res Perspect 2015;6(3):211-217.
Published online June 30, 2015
DOI: https://doi.org/10.1016/j.phrp.2015.05.003
In 2008, Korea Centers for Disease Control and Prevention initiated the first nationwide survey, Korea Community Health Survey (KCHS), to provide data that could be used to plan, implement, monitor, and evaluate community health promotion and disease prevention programs. This community-based cross-sectional survey has been conducted by 253 community health centers, 35 community universities, and 1500 interviewers. The KCHS standardized questionnaire was developed jointly by the Korea Centers for Disease Control and Prevention staff, a working group of health indicators standardization subcommittee, and 16 metropolitan cities and provinces with 253 regional sites. The questionnaire covers a variety of topics related to health behaviors and prevention, which is used to assess the prevalence of personal health practices and behaviors related to the leading causes of disease, including smoking, alcohol use, drinking and driving, high blood pressure control, physical activity, weight control, quality of life (European Quality of Life-5 Dimensions, European Quality of Life-Visual Analogue Scale, Korean Instrumental Activities of Daily Living ), medical service, accident, injury, etc. The KCHS was administered by trained interviewers, and the quality control of the KCHS was improved by the introduction of a computer-assisted personal interview in 2010. The KCHS data allow a direct comparison of the differences of health issues among provinces. Furthermore, the provinces can use these data for their own cost-effective health interventions to improve health promotion and disease prevention. For users and researchers throughout the world, microdata (in the form of SAS files) and analytic guidelines can be downloaded from the KCHS website (<ext-link ext-link-type="uri" xlink:href="http://KCHS.cdc.go.kr/" id="intref0015-main">http://KCHS.cdc.go.kr/</ext-link>) in Korean.

Antihematotoxic Role of Bunium persicum Seed Differential Extracts in Animal Model: Reactive Oxygen Species Might Be a Contributor
Tahir Ali, Kamran Saeed, Muhammad Abdullah, Iram Murtaza
Osong Public Health Res Perspect 2014;5(6):358-363.
Published online December 31, 2014
DOI: https://doi.org/10.1016/j.phrp.2014.11.005
Objectives
Humans have been using plants as natural medicines since prehistoric times. <i>Bunium persicum</i> is a rich source of oils with different biological activities such as antioxidative and antimicrobial activities. The aim of this study is to evaluate the antihematotoxic and antioxidative effects of the differential extracts of <i>B. persicum</i> against leukemic blood induced hematotoxicity in an animal model.
Methods
This study was performed on animals, which were divided into several groups: normal control, disease control, and groups that were administered with differential extracts of plants. We measured the concentration of free radical [reactive oxygen species (ROS)] and hematological parameters as blast cells from the tibia and femur in different groups.
Results
The ROS level and blast cells count were higher in disease control groups than in groups treated with varying amounts of <i>B. persicum</i> extract and the normal group. Moreover, there was an imbalance in hematological parameters in untreated and treated groups with a correlation between free radicals and plant extract administration.
Conclusion
These findings may indicate a possible link between free radicals and hematotoxicity and blast cells, while depicting a potential therapeutic role for <i>B. persicum</i> against ROS-induced hematotoxicity.

DOI: https://doi.org/10.1016/j.phrp.2014.10.006
Objectives
A fast and accurate diagnosis is necessary to control and eliminate tuberculosis (TB). In Korea, TB continues to be a serious public health problem. In this study, diagnostic tests on clinical samples from patients suspected to have TB were performed and the sensitivity and specificity of the various techniques were compared. The main objective of the study was to compare various diagnostic tests and evaluate their sensitivity and specificity for detecting tuberculosis.
Methods
From January 2013 to December 2013, 170,240 clinical samples from patients suspected to have TB were tested with smear microscopy, acid-fast bacilli culture, and real-time polymerase chain reaction (PCR). The test results were compared and data were analyzed.
Results
A total of 8216 cultures tested positive for TB (positive detection rate, 4.8%). The contamination rate in the culture was 0.6% and the isolation rate of nontuberculous mycobacteria was 1.0%. The sensitivity and specificity of smear microscopy were 56.8% and 99.6%, respectively. The concordance rate between the solid and liquid cultures was 92.8%. <i>Mycobacterium</i> isolates were not detected in 0.4% of the cases in the liquid culture, whereas no <i>Mycobacterium</i> isolates were detected in 6.8% of the cases in the solid culture. The sensitivity and specificity of real-time PCR for the solid culture were 97.2% and 72.4%, respectively, whereas the corresponding data for the liquid culture were 93.5% and 97.2%.
Conclusion
The study results can be used to improve existing TB diagnosis procedure as well as for comparing the effectiveness of the assay tests used for detecting <i>Mycobacterium tuberculosis</i> isolates.

Published online February 28, 2014
DOI: https://doi.org/10.1016/j.phrp.2013.12.001
Objectives
The present study aimed to investigate the prevalence of and factors associated with H1N1 preventive behaviors in a community-based population.
Methods
A cross-sectional study was conducted in three urban and two rural communities in Korea. Interviews were conducted with 3462 individuals (1608 men and 1854 women) aged ≥ 19 years during February–March 2010. Influenza-related information including anxiety, preventive behaviors and their perceived effectiveness, vaccination status, past influenza-like illness symptoms, and sources of and trust in information was obtained.
Results
Among 3462 participants, 173 reported experiencing influenza-like illness symptoms within the past 12 months. The mean H1N1 preventive behavior score was 25.5 ± 5.5 (out of a possible 40). The percent of participants reporting high perceived effectiveness and high anxiety was 46.2% and 21.4%, respectively. After controlling for potential confounders, H1N1 preventive behavior scores were predicted by a high (β = 3.577, <i>p</i> < 0.001) or moderate (β = 2.529, <i>p</i> < 0.001) perception of their effectiveness. Similarly, moderate (β = 1.516, <i>p</i> < 0.001) and high (β = 4.103, <i>p</i> < 0.001) anxiety scores predicted high preventive behavior scores.
Conclusion
Effective methods of promoting population behavior change may be nationwide campaigns through mass media, as well as education and promotion by health care providers and broadcasters.

Dietary Patterns and Osteoporosis Risk in Postmenopausal Korean Women
Seon-Joo Park, Seong-Eun Joo, Haesook Min, Jae Kyung Park, Yeonjung Kim, Sung Soo Kim, Younjhin Ahn
Osong Public Health Res Perspect 2012;3(4):199-205.
Published online December 31, 2012
DOI: https://doi.org/10.1016/j.phrp.2012.10.005
Objectives
The prevalence of osteoporosis and related fractures has increased rapidly in Korean women. Proper nutrition intake is associated with the prevention of osteoporosis. We analyzed the association between dietary patterns and the risk of osteoporosis during a 4-year follow-up in postmenopausal Korean women.
Methods
Postmenopausal women (<i>n</i> = 1,725) who participated in the Korean Genome and Epidemiology Study were enrolled. Food intake was assessed using a validated semiquantitative food frequency questionnaire, and a quantitative ultrasound device was used to measure the speed of sound at the radius and tibia.
Results
Three major dietary patterns were identified using factor analysis based on baseline intake data: traditional (high intake of rice, kimchi, and vegetables), dairy (high intake of milk, dairy products, and green tea), and western (high intake of sugar, fat, and bread). Multivariate Cox proportional hazards models were used to estimate relative risk for osteoporosis. An inverse association was detected between the dairy dietary pattern and the osteoporosis incidence [relative risk (RR): 0.63, 95% confidence interval (CI): 0.42–0.93, <i>p</i>-trend=0.055 in radius; RR: 0.56, 95% CI: 0.35–0.90, <i>p</i>-trend=0.048 in tibia]. Individuals in the highest quintile for the traditional dietary pattern (<i>p</i>-trend = 0.009 in tibia) and western dietary pattern (<i>p</i>-trend = 0.043 in radius) demonstrated a higher risk of osteoporosis incidence than those in the lowest quintile.
Conclusion
These results suggested that high consumption of milk, dairy products, and green tea may reduce the risk of osteoporosis in postmenopausal Korean women.
